The effect of the COVID-19 lockdown on mental health care use in South Africa: an interrupted time-series analysis

Aims
The coronavirus disease 2019 (COVID-19) pandemic and ensuing restrictions have negatively affected the mental health and well-being of the general population, and there is increasing evidence suggesting that lockdowns have led to a disruption of health services. In March 2020, South Africa introduced a lockdown in response to the COVID-19 pandemic, entailing the suspension of all non-essential activities and a complete ban of tobacco and alcohol sales. We studied the effect of the lockdown on mental health care utilisation rates in private-sector care in South Africa.
MethodsWe conducted an interrupted time-series analysis using insurance claims from 1 January 2017 to 1 June 2020 of beneficiaries 18 years or older from a large private sector medical insurance scheme. We calculated weekly outpatient consultation and hospital admission rates for organic mental disorders, substance use disorders, serious mental disorders, depression, anxiety, other mental disorders, any mental disorder and alcohol withdrawal syndrome. We calculated adjusted odds ratios (OR) for the effect of the lockdown on weekly outpatient consultation and hospital admission rates and the weekly change in rates during the lockdown until 1 June 2020.
Results710 367 persons were followed up for a median of 153 weeks. Hospital admission rates (OR 0.38; 95% confidence interval (CI) 0.33–0.44) and outpatient consultation rates (OR 0.74; 95% CI 0.63–0.87) for any mental disorder decreased substantially after the introduction of the lockdown and did not recover to pre-lockdown levels by 1 June 2020. Health care utilisation rates for alcohol withdrawal syndrome doubled after the introduction of the lockdown, but the statistical uncertainty around the estimates was large (OR 2.24; 95% CI 0.69–7.24).
ConclusionsMental health care utilisation rates for inpatient and outpatient services decreased substantially after the introduction of the lockdown. Hospital admissions and outpatient consultations for alcohol withdrawal syndrome increased after the introduction of the lockdown, but statistical uncertainty precludes strong conclusions about a potential unintended effect of the alcohol sales ban. Governments should integrate strategies for ensuring access and continuity of essential mental health services during lockdowns in pandemic preparedness planning.

Objectives:
This review systematically explores the current available evidence on the effectiveness of interventions provided to first responders to prevent and/or treat the mental health effects of responding to a disaster.
Methods:A systematic review of Medline, Scopus, PsycINFO, and gray literature was conducted. Studies describing the effectiveness of interventions provided to first responders to prevent and/or treat the mental health effects of responding to a disaster were included. Quality was assessed using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) criteria, and the Critical Appraisal Skills Programme (CASP) checklist.
Results:Manuscripts totaling 3869 met the initial search criteria; 25 studies met the criteria for in-depth analysis, including 22 quantitative and 3 qualitative studies; 6 were performed in low- and middle-income countries (LMICs); 18 studies evaluated a psychological intervention; of these, 13 found positive impact, 4 found no impact, and 1 demonstrated worsened symptoms after the intervention. Pre-event trainings decreased psychiatric symptoms in each of the 3 studies evaluating its effectiveness.
Conclusions:This review demonstrates that there are likely effective interventions to both prevent and treat psychiatric symptoms in first responders in high-, medium-, and low-income countries.

Crop yield loss–weed density relationships critically influence calculation of economic thresholds and the resulting management recommendations made by a bioeconomic model. To examine site-to-site and year-to-year variation in winter Triticum aestivum L. (winter wheat)–Aegilops cylindrica Host. (jointed goatgrass) interference relationships, the rectangular hyperbolic yield loss function was fit to data sets from multiyear field experiments conducted at Colorado, Idaho, Kansas, Montana, Nebraska, Utah, Washington, and Wyoming. The model was fit to three measures of A. cylindrica density: fall seedling, spring seedling, and reproductive tiller densities. Two parameters: i, the slope of the yield loss curve as A. cylindrica density approaches zero, and a, the maximum percentage yield loss as A. cylindrica density becomes very large, were estimated for each data set using nonlinear regression. Fit of the model to the data was better using spring seedling densities than fall seedling densities, but it was similar for spring seedling and reproductive tiller densities based on the residual mean square (RMS) values. Yield loss functions were less variable among years within a site than among sites for all measures of weed density. For the one site where year-to-year variation was observed (Archer, WY), parameter a varied significantly among years, but parameter i did not. Yield loss functions differed significantly among sites for 7 of 10 comparisons. Site-to-site statistical differences were generally due to variation in estimates of parameter i. Site-to-site and year-to-year variation in winter T. aestivum–A. cylindrica yield loss parameter estimates indicated that management recommendations made by a bioeconomic model cannot be based on a single yield loss function with the same parameter values for the winter T. aestivum-producing region. The predictive ability of a bioeconomic model is likely to be improved when yield loss functions incorporating time of emergence and crop density are built into the model's structure.

Three models that empirically predict crop yield from crop and weed density were evaluated for their fit to 30 data sets from multistate, multiyear winter wheat–jointed goatgrass interference experiments. The purpose of the evaluation was to identify which model would generally perform best for the prediction of yield (damage function) in a bioeconomic model and which model would best fulfill criteria for hypothesis testing with limited amounts of data. Seven criteria were used to assess the fit of the models to the data. Overall, Model 2 provided the best statistical description of the data. Model 2 regressions were most often statistically significant, as indicated by approximate F tests, explained the largest proportion of total variation about the mean, gave the smallest residual sum of squares, and returned residuals with random distribution more often than Models 1 and 3. Model 2 performed less well based on the remaining criteria. Model 3 outperformed Models 1 and 2 in the number of parameters estimated that were statistically significant. Model 1 outperformed Models 2 and 3 in the proportion of regressions that converged on a solution and more readily exhibited an asymptotic relationship between winter wheat yield and both winter wheat and jointed goatgrass density under the constraint of limited data. In contrast, Model 2 exhibited a relatively linear relationship between yield and crop density and little effect of increasing jointed goatgrass density on yield, thus overpredicting yield at high weed densities when data were scarce. Model 2 had statistical properties that made it superior for hypothesis testing; however, Model 1's properties were determined superior for the damage function in the winter wheat–jointed goatgrass bioeconomic model because it was less likely to cause bias in yield predictions based on data sets of minimum size.

The ecological significance of species diversity within agroecosystems has become a point of interest in recent years. Although the time and spatial scale at which diversity is measured may influence the interpretation of its functional importance, little research has been conducted on methodological approaches to assess the number and relative abundance of agricultural plant species. In this study, we (1) evaluated the applicability of the species-area curve to examine plant species richness and α and β diversity in conventional no-tillage and organically managed spring wheat systems, and (2) assessed temporal changes in plant species richness across systems. Measurements were obtained at three times during two growing seasons in experimental plots and at three times during one growing season on commercial farms in Montana. In accordance with previous studies, management system affected species richness and diversity. In eight of nine studied cases, we detected a positive relationship between species richness and sampled area. In these eight cases, intercepts (α diversity) were higher for the organic systems than for the conventional no-tillage systems. Slopes (β diversity) were higher for the organic system in six of nine cases studied. Species richness declined as the season progressed for both systems, with the organic systems consistently having more species than the conventional no-tillage systems. Despite differences in the species composition and between the experimental plots and commercial farm field size, the patterns of species richness and α and β diversity were relatively constant, suggesting that the processes responsible for the assembly of plant communities in agroecosystems of the Northern Great Plains are similar at a range of spatial scales.

The overall objective of design against fatigue is to achieve adequate safety and reliability for minimum structural weight and, having this end in view, the fatigue performance of an aircraft structure is a major consideration in the design, testing and operation of an aircraft. This has not always been the case, indeed until the early 1950s the design of aircraft structures was based almost entirely on strength considerations with little regard to fatigue. However, in the decade following the Second World War a series of accidents both in the military and civil field, culminating in the Comet disasters, focused attention on the need to consider the problem of fatigue in design and provided the climate of opinion to support requirements for much more extensive and expensive full scale fatigue testing than had been contemplated hitherto. Since that time, advances in safety and reliability have been achieved in the face of customer requirements for major increases in overall fatigue life and for higher standards of day-to-day reliability of the structure. For example, in the military field a required fatigue life exceeding 6000 hours would not be exceptional now for a combat aircraft (Fig. 1) as compared with a life in service of about 1500 hours for such an aircraft in the years immediately following the Second World War. On the civil side, whereas lives of about 20 000 hours would have been considered adequate in the early 1950s, operators now require lives in excess of 60 000 hours. The European Airbus shown in Fig. 2 typifies such aircraft currently in service. Both military and civil aircraft are now expected to be in service for 20 or more years.

In June, 1962, a type of bug injury was found by the junior author to be prevalent in a well cared for apple orchard of 800 trees at Debec, Carleton County, N.B. Adults of the bug causing the injury were collected in early July and determined by Dr. L. A. Kelton, Entomology Research Institute, Ottawa, as Lygidea mendax Reut., commonly known as the apple redbug. This is the first record of this species in New Brunswick.

Observations have been made on the ultrastructure of objects identified as ovarian balls from female Moniliformis after 1, 3, 5, 7 and 9 days in the rat. Similar objects are present in the body cavity of female cystacanths from cockroaches. The ovarian balls from 1-day-old worms are formed of several cells which are enclosed within a surface coat. During development, the ovarian balls increase in size and cell number and the internal organization appears to become more complex. After 9 days of development in the rat, the surface coat of the ovarian balls can no longer be observed and microvilli are present. It also appears that the supporting syncytium has been formed by this time.

A number of methods have been developed for synchronization of oestrus in sheep (reviewed by Maxwell, 1984), but the most successful have been based on the suppression of the oestrous cycle by treatment with a polyurethane progestagenimpregnated intravaginal sponge (Robinson, 1965). A precise synchronization of oestrus and ovulation for artificial insemination (A.I.) is obtained following a 12- to 14-day treatment with the intravaginal sponge and an injection of pregnant mares serum gonadotrophin (PMSG) at sponge removal (Robinson & Smith, 1967). This allows a single fixed time A.I. to be performed on all ewes without the need for detection of oestrus.

We describe a malignant lymphoma of the parotid gland arising in a patient with Sjögren's syndrome. Diagnosis was established on needle biopsy which showed a mixed population of lymphoid cells. Immunohistochemistry revealed B-lymphoid cells, T-lymphoid cells and histiocytes. Clonal immunoglobulin heavy chain gene rearrangement was demonstrated using the polymerase chain reaction. Within the confines of the small biopsy, the lesion qualifies for the designation T-cell-rich histiocyte-rich B-cell lymphoma. The value of molecular techniques in the diagnosis of malignant lymphoma on limited tissue samples is highlighted by this case.
A note on the effect of post-mating stocking rate on reproductive performance of Greyface ewes

Continuously grazed rye grassl clover swards with surface heights of 8 to 10 cm at 5 weeks before a synchronized mating and 7·5 cm at mating were grazed by 151 Greyface ewes stocked at 12 per ha until mating. The effects of two stocking rates (eight and 16 ewes per ha) for 6 weeks over the mating and post-mating period were then studied on live weight, body condition and reproductive peformance. Sward height fell more rapidly post mating when stocked at 16 ewes per ha than at eight ewes per ha, but remained above 3·5 cm until 4 weeks after first mating under both rates. Ewes stocked at 16 per ha became significantly lighter and leaner than ewes stocked at eight per ha. There was no significant effect of post-mating stocking rate on reproductive performance in terms of conception rate and lambing rate to first mating or lambing rate to all matings. Reproductive performance of Greyface ewes is therefore unlikely to be adversely affected by post-mating stocking rate on swards of 7 to 8 cm which do not fall below 3·5 to 4·0 cm until 3 to 4 weeks after mating.

One problem encountered in the widespread application of sheep artificial insemination (AI) is the rapid decline in fertility of fresh or liquid chilled ram semen. An effective deep-freezing procedure would enable semen to be collected and stored from selected rams throughout the year for subsequent use in controlled breeding programmes, as well as facilitating the import and export of semen.
Following some encouraging results from the use of pooled semen frozen by an Australian pellet method (Maxwell et al 1980), further field trials were conducted to investigate fertility of semen frozen from individual rams as well as pooled semen. The effects of season of freezing and number of inseminations on fertility were also investigated.
